COACH Program Eligibility
To be considered for the next cohort of the COACH Program, your team should meet the following guidelines. If you have questions or would like to be considered please contact Shor at email@example.com.
Team must be an alumni of the Community Teams Program
Team needs to have completed Community Teams Program in the last six years (completed after 2011)
Team should be committed to prioritizing policy, systems, and environmental change work
COACH Team Application Form (one per team)
Team Member Form
All team members should complete the Team Member Form. Each team member should describe what they can contribute to the team and coalition/partnership and what they would like to gain by participating in the COACH Program.
One team member should indicate that they are the primary contact for your team. Primary contact responsibilities include:
- Serving as communication point person and liaison with HWLI
- Scheduling and coordinating site visits
- Ensuring team members are participating in CTP activities
- Serving as contact person for 20-month follow-up evaluation
